receiving additional care, either following a stay in a medical institution or

to avoid hospitalization. A registered caregiver and medical experts provide

home health care, allowing patients to stay independent and retain their

A home health aide is a professional caregiver who offers basic personal

care and health-related services to a range of patients, often the elderly or

disabled, who need daily support. Home health aides work in the homes of

clients or residential facilities. Home health aides must complete a formal

training program and be qualified or registered. Registered nurses and

other medical professionals supervise these professionals.


Home care eligibility depends on your medical needs, financial status, and home

care agency, insurance provider, and government program standards.

General eligibility factors include:

Medical Need: Your health determines eligibility. Home care may be available

if you're recovering from surgery or sickness, have a chronic ailment, or have

physical or cognitive disabilities that make daily chores challenging. A doctor

must approve home care.

Financial Resources: Insurance or other programs may cover a portion of the

cost of expensive home care services. Medicaid covers home health services for

low-income people. Long-term care insurance may cover home care. Medicare

may fund short-term home care under certain conditions.

No. Your Medicare benefit will cover all costs associated with Home

Health Care.


If you are not being discharged from the hospital or nursing home,

your doctor needs to submit an order for home care services. If

necessary, we can assist you with the process.

be bedridden. Under Medicare, a patient is considered "homebound" if he or she cannot leave the house without "considerable and taxing

effort".

The PDA Waiver offers an extensive range of home care and community based services to senior individuals who require nursing facility care but prefer to remain in their own homes or in a community living domicile. This waiver is also referred to as the PA Council of Aging Waiver, the PDA Waiver, the Office of Long Term Living (OLTL) Waiver, and the Home and Community-Based Services (HCBS) Waiver for Individuals Aged 60 and Over. 

This program covers basic personal care services and support to help family members pay for services such as adult day care and respite care. Additionally, the program pays for home medical equipment as well as home modifications to improve accessibility such as wheelchair ramps and stair glides. Under this waiver, individuals can choose from which vendors they purchase certain goods or self care services. 

Services provided under this waiver are determined on a needs basis, and there is no limit on how many can be received by a single beneficiary.


Adults aged 19- 64 and up years old are eligible to apply through Pennsylvania State of Health. Under managed care, Medicaid beneficiaries are entitled to join to  ReliableCare USA Home care community.
